Follow the instructions in the LOG Guide when responding to each of this Log Entry Form’s 4 sections.

(Note: It is recommended that you print the LOG Guide document to make it easier to read while you are entering responses to each of the four sections shown below.)

 

When you enter responses to each Section, preface the response with the date of entry (e.g.- 091513) and the consecutive number for that particular day if there is more than one per day (e.g. 091513-2). SAVE ALL ENTRIES BEFORE LEAVING THIS TEMPLATE.
